The symposium presentations revealed the considerable maturation of the field of acupuncture research, yet two provocative paradoxes emerged. First, a number of well-designed clinical trials have reported that true acupuncture is superior to usual care, but does not significantly outperform sham acupuncture, findings apparently at odds with traditional theories regarding acupuncture point specificity.
Second, although many studies using animal and human experimental models have reported physiological effects that vary as a function of needling parameters e. This White Paper, collaboratively written by the SAR Board of Directors, identifies gaps in knowledge underlying the paradoxes and proposes strategies for their resolution through translational research.
Such a strategy, incorporating considerations of efficacy, effectiveness and qualitative measures, will strengthen the evidence base for such complex interventions as acupuncture. In Novemberthe Society for Acupuncture Research SAR held an international symposium aimed at presenting and discussing the progress made in acupuncture research during the decade following the NIH Consensus Development Conference.
The symposium presentations, as well as their published summaries [ 2 — 4 ], unequivocally showed that the field of acupuncture research has significantly expanded and matured since However, intriguing paradoxes emerged during the symposium and the writing of the summaries. Proceedings from our conferences as well as syntheses of our findings from systematic reviews were discussed at a structured SAR Board workshop in October, held at Georgetown University and facilitated by Aviad Haramati, a non-SAR member.
The workshop resulted in the articulation of two primary paradoxes as well as an outline for this White Paper. The goal of this White Paper is not to present a detailed review of the literature. Rather, this paper aims to identify gaps in current knowledge that underlie these paradoxes and to recommend research strategies that can help deconstruct and resolve them. With this approach, findings from clinical trials will better inform the role of acupuncture in our evolving health care system.

The journal will accept original basic or clinical research papers, worthwhile reviews on safe and effective therapeutic methods, applications, limitations, theories, or principles of acupuncture and electro-therapeutics, including electro-analgesia or any method of treatment through the application of various types of electric or electromagnetic field stimuli as well as electrical and nonelectrical diagnostic methods.
The journal will also consider serious research on any related methods of treatment such as Shiastsu, moxibustion, herbal medicine, and standard and experimental stages of Western medicine, with physiological and pharmacological studies. In general, submitted papers must be based either on the documentation of data through scientifically proven or accepted methods, or on the explanation of new theories in the field with critical evaluation of such ideas. For example, papers may present data such as the effects of acupuncture on the cardiovascular or nervous systems using blood chemistry, hemodynamics, enzymes, and other biochemical or pathophysiological, Bi-Digital O-Ring Test, nerve conduction velocity, threshold stimulation for pain, somatosensory evoked potentials or any other electro-physiological method that is scientifically acceptable.
Papers not dealing directly with acupuncture, electro-therapeutics, or the related methods of diagnosis or treatment mentioned above will also be considered if they have important implications for either improving diagnostic methods or treatment, or elucidating the underlying principles or the history or medicolegal aspects of these fields.

One therapy that has received considerable attention for the treatment of chronic pain is acupuncture. Thirteen RCTs met eligibility criteria. Individual patient data was received from 10 RCTs, representing 2, patients. When combined with the research, the new meta-analysis incorporated data from 20, patients over 39 trials. A meta-analysis published in the Journal of Pain has concluded that acupuncture is effective for a number of chronic pain conditions.
Results indicated that the effects of acupuncture were persistent over time, and that the benefits could not be explained away by the placebo effect. Family physicians may not feel comfortable prescribing these treatments without receiving guidance and training in the benefits, harms and indications. More than half of the RCTs 23 used acupuncture based on traditional Chinese techniques; the rest used Western techniques or a combination of the two.
In nine RCTs, acupuncture points were selected according to a fixed needle formula i. In 80 percent of the RCTs, patients underwent between six and 15 acupuncture sessions. Most patients received acupuncture one to two times per week.Read it forward
In slightly more than half of the RCTs, the typical acupuncture session lasted less than 30 minutes. These effects appeared to persist for at least 12 months after receiving acupuncture. The authors added that "the effects of acupuncture are not completely explicable in terms of placebo effects," but cautioned that "factors other than the specific effects of needling at correct acupuncture point locations" contribute to the benefits derived from receiving acupuncture.
The authors addressed the issue of effect size and relevance. They argued that relevance should be determined by comparing acupuncture with no-acupuncture control because the decision physicians make to treat or refer patients "is not between acupuncture and sham but between acupuncture and no acupuncture. They suggested that acupuncture would be "a reasonable option" to consider in patients with chronic pain, and called for additional research to determine the best way of incorporating acupuncture into the care of these patients.Researchgate articles in italy time
